Keep in mind that when you fill out an enrollment form on line a parent will still need to sign a release and waiver form at the gym.
Each student must register by completing an application form and a release form. Family members with the same address may fill out one
application form. The release form must be signed by BOTH parents. You will only be notified if your first choice class is full. If your child does not start the first week, we will assure a smooth transition into the proper class and the cost will be pro-rated. Parents and visitors are welcome to observe from our second floor only. Parkettes is not responsible for items left in your vehicle.
ATTIRE: A Parkettes leotard is the preferred attire for girls; Parkettes gym shorts and t-shirt for boys. These items can be purchased in ourgift shop “The Chalk Box”. Children should remove all jewelry and tie shoulder length hair back.
WARM-UP: This 10-15 minute activity begins each class. It helps students to develop flexibility and strength while safely preparing for
gymnastics (sometimes serving as an emotional adjustment to class). Please make every effort to arrive on time.
MAKE-UP: Each child is permitted one make-up per semester for ILLNESS only. Please call after your child has missed a class to schedule.
MEMBERSHIP FEE: This is due upon enrollment, and is effective through August 31, 2011. This fee is not included in the class fee.
($35.00 per person, $20.00 each additional family member) Fees may be paid by MASTERCARD, VISA, DISCOVER, AMERICAN
EXPRESS, check or cash. Payments may be given at the office, dropped in the payment box located outside the office or sent by mail.
Make checks payable to PARKETTES and write the name of person attending class, day, time and phone number on each check.
ALL PAYMENTS ARE NON-REFUNDABLE: A service charge of $20.00 will be assessed for each RETURNED CHECK.
A monthly FINANCE CHARGE OF 1.5% will be added to the balance owed if payment is not received by:
1st semester: October 12, 2011 • 2nd semester: January 14, 2012 • 3rd semester: April 14, 2012
TWO PAYMENT PLAN: Due 1st and 5th weeks (add $5.00 to each payment) total $10.00.
BONUS PAYMENT: $5.00 off when paid in full by:
1st semester: August 17, 2011 • 2nd semester: November 12, 2011 • 3rd semester: February 18, 2012
FAMILY DISCOUNT AVAILABLE - only applies to full semester payments:
1st person: Full rate • 2nd person: 5% off lowest price • 3rd person: 10% off lowest price
The Family Discount is not available on line. Please call us to receive that discount.
